I WANT TO CONTINUE TO ACCESS MY COMMUNITY INDEPENDENTLY WITH 1:1 SUPPORT SO THAT I CAN CONTINUE TO TAKE ADVANTAGE OF NEW OPPORTUNITIES TO VARY MY ROUTINES AND EXPERIENCE NEW THINGS.

My main support goals:

  • I want to share ordinary places that define community life involvement in everyday settings
  • I needsupport for daily opportunities to be involved in my community
  • I want to choose my support worker for 1.1 support
  • I need 1.1 support to become more independent of my family
  • Increase my understanding ofthe community environment
  • I want to build a circle of support
  • I want a supported active lifestyle, busy, fun day
  • I want toincrease social & recreational activities
  • I want to experience new things
  • I want a lifestyle of choice
  • I want to develop relationships and connections
  • I want to develop interests, confidence and skills
  • I want life long learning from a community setting
  • I want to develop better social skills/be less impatient

  • I want to continue my ten pin bowling
  • I want to linkup with friends on occasions
  • I want to go shopping and experience being a customer
  • I want to improve my sensory processing reactions- be less impatient

The goal of Active Support is to ensure that people even with the most significant disabilities have ongoing, daily support to be engaged in a variety of life activities and opportunities of their choice.
